2.03 INSTRUCTOR GUIDANCE – CONTEXT STATION

Objectives: Manage mini-scenarios involving airway procedures

Setup: - Oxygen system with BVM, Nasal Cannula, NRB, OPA/NPA, - EMT manikin with airway capability - Suction unit

Instructions to be read to students: “I am going to ask you to manage a patient with various conditions. Here is your vignette:”

Precautions: This allows students to put the individual skills into context as a “putting it all together” station. You can have students work in teams of 3. If you have enough equipment, run two simultaneous stations of three students each. This is their prep for BVM APNEIC PATIENT skills station, so be thorough – their success will likely depend on your training in this station.

The scenario guidance is on the skills sheet – read the EXAMINER pieces and make sure the students go through that exercise several times.

Debriefing: What are some good starting parameters for ventilation of patients?

If patients breathe <8 or >24 with symptoms, they need ventilation

What should be done when patients don’t tolerate the oral airway?

Patients who do not tolerate the oral airway should get a nasal airway unless there are contra- indications

What is an example of a patient who needs suctioning?

Patients who vomit need suction. This is usually indicated by gurgling

What is an example of a patient who needs an airway device?

Patients who have upper airway relaxation need an OPA/NPA. This is usually indicated by unresponsiveness and/or snoring
