Intubation may be appropriate when: Persistently hypoxic No gag reflex Danger of losing the airway Rapidly decreasing LOC Ventilate to ETCO2 of 35 and 20 breaths per minute if signs of herniation are present.

Consider Versed 10mg IM
Ventilate at 20 bpm until dilated pupils constrict
